Low Testosterone: What Testing Comes Before TRT?

Symptoms alone do not diagnose low testosterone. Here is what to discuss before starting testosterone replacement therapy.

Testosterone replacement therapy, or TRT, should follow an appropriate diagnosis. For men, that generally means symptoms consistent with testosterone deficiency and repeat blood tests showing low testosterone. One result or a symptom checklist is not enough to decide whether treatment is appropriate.

Symptoms are a starting point

Reduced sex drive, changes in erections, low energy, and reduced muscle mass can prompt a conversation about testosterone. They can also overlap with other health concerns. Describe when the changes began, whether they are persistent, and what else changed around the same time. Bring your medication list and previous laboratory reports rather than relying on a remembered result.

Why morning testing and repeat results matter

Testosterone varies during the day and can change with illness and other factors. The Endocrine Society recommends confirming a low result with repeat morning fasting testing. A clinician interprets the results using the laboratory’s methods and your symptoms. If a deficiency is confirmed, the next question is why it is occurring; further evaluation may be needed before choosing treatment.

Discuss fertility before a prescription

TRT can suppress sperm production. Tell the clinician if you are planning children, even if pregnancy is not an immediate goal. The Endocrine Society recommends against starting testosterone therapy in men planning fertility in the near term. Do not treat this as a detail to revisit after treatment has started.

Monitoring is part of the treatment decision

A treatment discussion should cover expected benefits, possible adverse effects, relevant medical risks, and the follow-up plan. Depending on the patient, monitoring can include testosterone, blood counts, and prostate assessment. Ask who reviews the results and what would trigger a change or a stop. The plan should track how you feel as well as the measurements.

What to ask at a TRT consultation

Ask: What evidence supports the diagnosis? Could another condition explain these symptoms? What are the alternatives? How will we assess whether treatment is helping? What costs are included, and which are separate? A clear explanation is more useful than a promise to reach a particular “optimal” number.

Frequently asked questions

Can I start TRT because I feel tired?

Fatigue alone does not establish testosterone deficiency. A clinical evaluation should determine the appropriate next step.

Does a consultation guarantee a TRT prescription?

No. The decision depends on the evaluation, diagnosis, and whether treatment is appropriate for you.
